Surprisingly, the simplest things are what cause it.

Usually, it is sand or dust interfering with the lens extension mechanism. Another reason is if the camera has been dropped with the lens extended. Or the battery ran down with the lens extended.

Believe it or not, camera cases are BIG contributors to lens errors. The accumulation of sand, gunk, case fibers, and other materials rolling around the bottom of the case are materials thin enough to make their way into the camera lens. These materials love to cling to the camera by electrostatic build-up from the camera rubbing against the side of the case (especially those cases with soft fibrous interiors). Once these materials work their way into the lens mechanism problems start to occur.

As cameras are put together with very tight fittings and sensitive materials disassembling the camera takes expert handling and special tools in order to not cause further damage.
